As you might expect, World Balloon® requires certain information when we conduct our flight operations. Below is the form we use in our flight operations:
This document provides information on what to expect, how to prepare,
and things you can and can't do before and during the flight.It also includes
our release of liability and passenger acknowledgement of personal responsibility.
As it's name implies, it includes the release of liability that you will
be required to sign on flight day:

Each package sent out with flight certificates includes a copy of this document for your review. There is no need to fill out the document ahead of time, we will have it for you to sign during check-in on flight day.
Please download and review these document prior to our flight so that you are familiar with it. We encourage you to ask any questions that come up in reviewing this document prior to flight day.
